What it is

The dorsal hand is the back of the hand, where soft tissue covers tendons and veins. With age, the fat pad thins and the skin loses elasticity. The result is more visible veins, tendons, and bone structure.

The change is one of the most age-revealing features. The face can be carefully maintained while the hands tell the actual age. Volume restoration smooths the surface and brings the hands back in line.

What Causes It

Three factors drive hand volume loss.

Fat pad atrophy is the primary cause. The dorsal hand fat thins gradually with age.

Sun damage degrades collagen and elastin, accelerating skin laxity.

Bone resorption in the small bones of the hand subtly changes the underlying structure.

Most clients first notice their hands looking older in photos. Veins read more prominent. Tendons show through the skin more clearly. The skin appears thinner or more crepe-textured.

The change often becomes obvious when the face has been well-maintained. The contrast between face and hands becomes visible.

Hand aging accelerates in the forties and fifties. Sun exposure plays a larger role than for most facial areas because hands often receive significant unprotected sun across decades.

Stable weight, sun protection, and consistent moisturization slow the rate. Treatment becomes the leverage when changes have accumulated.

How It's Commonly Addressed

The industry treats hand volume loss with HA filler, Radiesse, or Sculptra placed in the dorsal hand. Each delivers a different result profile. Some clinics also recommend laser or IPL for surface concerns (age spots, texture).

How we approach hand restoration

For dorsal hand volume, we use HA filler (Restylane Lyft is common), Radiesse for more structural restoration, or Sculptra for gradual collagen rebuilding. Product choice depends on the client's preferences and the degree of volume loss.

Technique is precise. We use cannula placement to minimize bruising and ensure smooth distribution. Most plans use one to two syringes per hand for initial treatment.

For surface concerns (sun spots, texture, skin quality), we may pair volume work with laser like BBL or IPL on the dorsal hand. The combination delivers a more complete result than volume alone.
