Jowls aren't always treated where they appear. The right answer is often higher up the face.

Jowls are descended soft tissue along the jawline. They appear when the midface deflates and the cheek tissue migrates downward, pooling at the jawline ahead of the jaw angle.
The visible change is at the jawline, but the cause is usually higher up the face. This is why direct jowl treatment is often not the right move.
Clients come in describing their jawline as soft or undefined. They want it sharper. Many have been told elsewhere that filler in the jawline is the answer. We give a second opinion that often involves looking at the cheek and the pre-jowl support first.
Three forces produce jowls.
Cheek volume loss is the most common cause. As the midface deflates, the cheek tissue descends along gravity to the jawline.
Skin laxity reduces the elastic support that previously held tissue in place.
Bone resorption at the mandible and chin changes the underlying scaffold, contributing to descent.
Jowls usually become visible in the forties. The clean line of the jaw softens. A small pouch develops on either side of the chin, ahead of the jaw angle.
Some clients first notice it in profile photos. Others see it in the way light falls along their jawline at certain angles.
Jowls deepen progressively through the forties and fifties. The rate depends on cheek volume status, skin quality, and genetics.
Significant weight loss can accelerate jowl appearance by removing supporting volume. Stable weight tends to slow the rate.
The industry approaches jowls with filler directly at the pre-jowl area or along the jawline. Voluma, Restylane Lyft, and Radiesse are common products. Some clinics also recommend Sculptra for collagen rebuilding.
The industry problem is treating the symptom (jowl) without addressing the cause (cheek deflation). The result can look heavier or unnatural.
Jowls treatment starts with assessment. Cheek volume comes first. Pre-jowl support comes second. Direct jowl treatment comes last and is conservative.
We use firm structural filler like Voluma, Restylane Lyft, or Radiesse. Sculptra builds collagen over months when long-term restoration is the goal. Dosing varies. A modest correction may use one to two syringes total across the lower face.
For very advanced jowls with significant skin laxity, surgical options may deliver a better result than filler alone. We'll be honest about that.
