What it is

Lip volume loss is the gradual thinning of the upper and lower lip with age. By the fifties, most clients have lost roughly 40% of their lip volume compared to their twenties. The vermilion border softens. The cupid's bow flattens. Vertical smoker's lines appear.

The cumulative effect reads as a more closed, less defined mouth. Restoration aims to recover what was lost, not to add what wasn't there.

What Causes It

Three forces drive lip volume loss.

Tissue atrophy is the primary cause. The lip itself thins gradually with age.

Bone resorption around the mouth changes the underlying scaffold, contributing to the appearance of thinning.

Sun damage and smoking accelerate every part of this. Clients with significant sun exposure or smoking history often experience earlier and more pronounced volume loss.

Common Signs

Most clients first notice that their lipstick bleeds or feathers around the lip border. The smooth surface that used to hold color now traps it in fine lines.

The vermilion border softens before the lip body thins. The cupid's bow flattens. Clients describe their mouth looking smaller in photos than it does in the mirror.

Why It Changes Over Time

Lips lose volume gradually starting in the mid-thirties. The border softens, the cupid's bow flattens, and vertical lines develop on the upper lip from years of repeated mouth movement. The mouth corners begin to turn downward. The philtrum lengthens.

The pace depends on genetics, sun exposure, and smoking history. Some clients experience significant changes in their thirties; others retain volume into their fifties.

How It's Commonly Addressed

The industry treats lip volume loss with HA filler in the lip body, vermilion border, and sometimes the mouth corners. Restylane Kysse, Juvederm Volbella, Juvederm Vollure, and Skinvive are common choices.

The industry problem is over-filling. Lips are where filler is most often overdone. Faces with too much lip filler develop the overinflated look that everyone tries to avoid.

How we approach lip volume

Conservative dosing is non-negotiable. Most first lip treatments use less than a full syringe. We'd rather you come back in two weeks for more than overshoot in one visit.

Product choice matters. Restylane Kysse, Juvederm Volbella, and Juvederm Vollure all behave differently in lip tissue. Kysse holds shape and reads firm. Volbella is soft and integrates smoothly for hydration and subtle volume. Vollure handles more structural work. We choose by what the lip in front of us needs.

Symmetry is studied, not assumed. Most faces are slightly asymmetric, and forcing symmetric injection often makes the asymmetry more obvious. We map the lip you actually have before deciding where product goes.
