Marionette lines run vertically from the corner of the mouth downward toward the jawline. They get their name from the appearance of a marionette puppet, whose jaw articulates along similar lines.
The lines deepen the look of facial aging significantly. Even mild marionette lines can make a face read tired, sad, or older.
Clients come in describing themselves as looking tired, sad, or older in photos. They want to lift the mouth corners and soften the lines without changing the smile mechanics.
Three forces deepen marionette lines.
Volume loss in the cheek and around the mouth allows the tissue to descend, deepening the lines.
Repeated expression from frowning, speaking, and eating creates the natural crease.
Skin laxity and bone resorption change the supporting structure over time.
Marionette lines usually become visible in the forties. The mouth corners begin turning slightly downward at rest, and vertical lines appear extending toward the jaw.
Most clients first notice it in photos. The lines pull the face into a frown even when the expression is neutral.
Marionette lines deepen progressively through the forties and fifties. Volume loss in the cheek and lower face accelerates the process. Skin laxity adds to the appearance.
Significant weight loss can accelerate the lines. Stable weight, sun protection, and consistent skincare slow the progression.
The industry approaches marionette lines with HA filler placed in or alongside the line, sometimes supplemented with filler at the cheek for structural support. Vollure, Restylane Refyne, and RHA 3 are common.
Some clinics add small amounts of Botox to the depressor anguli oris, the muscle that pulls the mouth corner downward. This can help lift the corner subtly.
We start with structural assessment. Cheek volume often needs restoration first. The cheek supports the tissue above the mouth corners.
For the lines themselves, we use soft HA fillers like Juvederm Vollure, Restylane Refyne, or RHA 3. Placement is precise, alongside the line rather than directly in the deepest part.
For clients with significantly downturned mouth corners, we may add small amounts of Botox in the depressor anguli oris muscle to relax the downward pull. The combination of filler and neuromodulator often delivers better results than either alone.
