What it is

The temples sit on either side of the forehead, between the eye and the hairline. The temporal fat pad provides the convex shape that keeps the temple full. As that fat pad deflates with age, the temple hollows and the upper face changes.

The change is subtle. Most clients don't notice it until they see a photo from a decade ago. The downstream effects, on brow position and how the eyes appear, are usually what brings them in.

Why Patients Seek Treatment

Most clients don't seek temple treatment directly. They come in because they look tired or gaunt, or because their lateral brow has dropped, and learn that the temples are the structural cause. Treatment here lifts the upper face without addressing the brow or eyes directly.

What Causes It

Two main factors drive temple deflation.

Temporal fat pad atrophy is the primary cause. Like other facial fat compartments, the temple thins gradually with age.

Weight changes accelerate the process. Clients who've lost significant weight quickly often have more pronounced temple hollowing earlier than age alone would predict.

Common Signs

Most clients first notice temple changes indirectly. They look tired or thinner in the upper face. The lateral brow has dropped, making the eyes look heavier. Sunglasses don't sit the way they used to.

The hollow itself is often only visible in certain lighting. Side photos show it more clearly than mirrors.

Why It Changes Over Time

Temple deflation accelerates in the forties. By the fifties, most clients have visible hollowing if they look closely. The rate depends on genetics, body composition, and weight history.

Clients with lean facial structure or who've lost weight quickly often see temple changes earlier than expected.

How It's Commonly Addressed

The industry treats temple hollowing with HA filler or biostimulator placed in the deflated fat pad. Voluma and Restylane Lyft are common HA choices. Sculptra is the common biostimulator for gradual restoration. Radiesse enters when more lift is needed.

The temple is an advanced injection area. The temporal vessels run along the surface, and placement matters significantly. Not every clinic is comfortable working here.

How we approach the temples

Temple injection is advanced work. We use deep needle technique against the periosteum or cannula placed in the fat pad layer, depending on anatomy and goals.

HA fillers like Juvederm Voluma and Restylane Lyft provide immediate structure. Sculptra builds volume gradually across a series of treatments and is often the choice for clients who want slower restoration. Radiesse occasionally enters when more lift is needed.

Dosing is generous compared to other facial areas. The temple holds significant volume. Most plans use half to one full syringe per side, sometimes more for clients with significant hollowing.
