What it is

TMJ pain, technically temporomandibular joint disorder (TMD), refers to pain or dysfunction in the temporomandibular joint where the jaw connects to the skull. The pain may be in the joint itself, in the muscles around the joint, or both.

The cause is often muscular: chronic clenching and grinding create persistent tension in the masseter and other jaw muscles, which pulls on the joint and produces pain. Less commonly, the joint itself has structural issues that require dental or oral surgery evaluation.

What Causes It

Several factors drive TMJ pain.

Muscular tension from chronic clenching or grinding is the most common cause. Masseter and temporalis muscle overactivity pulls the joint out of alignment.

Stress and anxiety drive jaw clenching, often without conscious awareness.

Bite issues can contribute, though most TMJ pain is muscular rather than structural.

Joint disorders including disc displacement or arthritis, are less common but require dental or surgical evaluation.

Trauma to the jaw can trigger persistent TMJ pain.

Common Signs

Pain or tenderness in the jaw, especially around the joint just in front of the ear. Clicking or popping sounds when opening or closing the mouth. Limited range of motion. Pain that radiates to the temples, ears, or neck. Headaches concentrated around the temples.

Symptoms often worsen with stress, chewing, or jaw movement.

Why It Changes Over Time

Without treatment, TMJ pain often persists or worsens. Chronic muscle tension and joint stress can lead to permanent changes in the joint and surrounding tissues.

With masseter Botox and stress management, most clients experience significant relief. Maintenance is typical.

How we approach TMJ pain

Masseter Botox is our primary tool for muscular TMJ pain.

We inject Botox into the masseter and sometimes the temporalis muscle. The muscle tension reduces over two to three weeks. Most clients see significant improvement in jaw pain, headaches, and clicking within fourteen days.

For clients whose TMJ pain has a structural joint component, we coordinate with dentistry or oral surgery for evaluation. Botox doesn't address true joint disorders.

We also recommend physical therapy with a TMJ-trained therapist alongside Botox. The combination produces better long-term outcomes than either alone.

Results last four to six months. Most clients schedule maintenance.
