Why the 11s form between the brows

The glabella sits between the brows, overlying the corrugator and procerus muscles. Every time you focus, squint, react, or concentrate, these muscles fire. The lines that form here, the 11s, are among the earliest visible expression lines on the face.

Glabella treatment is conceptually simple but technically demanding. The corrugator pulls the brows inward, the procerus pulls them downward. Treating both relaxes the area, but the placement and dose must respect the lift the surrounding frontalis muscle still needs to give. Too much, and the brow drops. Too little, and the lines stay.

Three muscles drive glabellar lines.

Corrugator supercilii originates near the medial brow and pulls the brows inward and downward when activated. It creates the vertical lines that read as the 11s.

Procerus sits at the bridge of the nose. When it fires, it pulls the medial brow downward, creating the horizontal line across the nose bridge.

Depressor supercilii assists the corrugator in pulling the brow down.

Treatment is the careful relaxation of all three to soften the lines without dropping the brow. Most plans use four to seven injection points distributed across the corrugator complex and procerus.

How this changes over time

The 11s are the first place expression turns into evidence.

Dynamic 11s show up first, often in the late twenties for clients who focus or frown frequently. By the mid-thirties, those lines begin to remain at rest. By the forties, they're often etched and may not fully release with neuromodulator alone.

This is why we treat preventively when we can. Stopping the muscle from contracting before the line etches keeps the surface smooth and reduces the depth of static lines that would have formed.

In Your 20s

For most clients, the 11s start to show up when concentrating, squinting, or frowning, but the skin returns to smooth at rest. This is the highest-leverage decade for preventive treatment.

In Your 30s

By the mid-thirties, the 11s start to stay etched even when the face is relaxed. Most clients first notice this in selfies or candid photos. Treatment shifts from purely preventive to a mix of softening and prevention.

In Your 40s

The 11s settle into more pronounced static lines. Neuromodulator continues to soften them, and small amounts of HA filler may be used to complete the smoothing of the deepest creases.

In Your 50s+

For lines that no longer fully release with neuromodulator, small amounts of HA filler placed precisely within the deepest 11s can finish the result. The goal stays the same: a relaxed forehead that reads rested.

How we treat it

The glabella is one of the most-treated areas in injectables, and one of the most often over-treated. The mistake is freezing the muscle completely, which can drop the brow and produce the frozen look that clients fear.

Our approach is calibrated relaxation. Most clients receive between fifteen and twenty-five units of Botox or equivalent dosing of Dysport or Daxxify, distributed across four to seven injection points. Heavier movers need more. Lighter movers need less. We dose to the muscle, not to a chart.

The glabella is also rarely treated in isolation. Treating it without considering the frontalis can drop the brow. We almost always think about the upper face as a system.

Results show up at four to seven days and reach full effect at two weeks. Most clients return every three to four months.
