Why marionette lines drag the face into a permanent frown

Marionette lines run vertically from the corner of the mouth toward the chin, named for the hinged appearance they create. They're driven by two forces: descent of the tissue around the mouth corner, and the muscle that actively pulls the corner downward.

Effective treatment addresses both. Filler restores the support that's been lost. Botox relaxes the depressor anguli oris muscle, which fights the descent.

Depressor anguli oris is a muscle that originates at the jawline and inserts at the corner of the mouth, actively pulling the corner downward.

Mouth corner support is provided by the surrounding fat compartments and bone. As both deflate, the corner descends without active muscle pull.

Treatment combines structural support with muscle relaxation.

In Your 30s

Some clients with active depressor muscles or genetic predisposition may notice mouth corner descent early. Treatment in this decade is uncommon and usually preventive.

This is when most clients first notice marionettes. The mouth corner has descended visibly. Treatment plans typically combine Botox and filler.

In Your 50s+

Marionettes are usually pronounced in this decade. Treatment combines depressor relaxation, mouth corner support, and selective filler in the line itself for the most natural correction.

How we treat marionettes

Marionette work is layered. We treat the depressor with two to four units of Botox per side, which relaxes the downward pull. We support the mouth corner with small amounts of HA filler placed precisely. For deeper grooves, we add soft filler along the line itself.

Product choice matters. We use Juvederm Vollure, Restylane Refyne, or RHA 3 here. Avoid heavier fillers, which can blunt the natural smile mechanics.

The result is subtle. The corner lifts a millimeter or two. The line softens. The face reads neutral instead of stern. Clients describe looking less tired or angry, not different.
