The hidden lift of the temples in upper-face aging

The temples sit on either side of the forehead, between the eye and the hairline. The area is anchored by the temporal fat pad, which loses volume gradually with age. As that fat pad deflates, the temples become hollowed, the eyes appear more sunken, and the lateral brow position drops slightly.

Treating the temples is one of the most quietly transformative moves in aesthetics. Most clients can't articulate what changed, only that they look more rested or more youthful. The change is structural, and it lifts the entire upper face.

Restoring temple volume with filler and biostimulators

The temple has three structural layers that matter for treatment.

Skin here is thin and shows hollowing readily.

Temporal fat pad beneath the skin provides the volume that keeps the temple convex. It deflates with age, allowing the area to hollow.

Temporalis muscle beneath the fat pad is a chewing muscle. The blood vessels that run along its surface make this an advanced injection area.

Treatment places volume back into the deflated layer. The technique matters: too superficial and the result looks puffy. Too close to a vessel and the risks rise. This is anatomic injector territory.

How this changes over time

Few clients ask for temple filler. Almost all of them love the result.

Temple deflation accelerates in the forties. By the fifties, most clients have visible hollowing if they look closely. The change is gradual, which is why most people don't notice until they see a photo from years earlier and the comparison is obvious.

Genetics, body composition, and weight changes all affect the rate. Clients who've lost weight quickly often have more pronounced temple hollowing earlier.

In Your 20s

Temples in this decade are full and usually require no treatment. Some clients with low body fat or specific facial structure may have hollowing earlier.

In Your 30s

Minor hollowing may begin, particularly in clients who've lost weight or have lean facial structure. Treatment in this decade is rare and reserved for genuine structural concerns.

In Your 40s

The temporal fat pad begins to deflate visibly. Lateral brow position may drop slightly. This is when most clients first consider temple treatment.

In Your 50s+

Significant hollowing is common in this decade. Treatment plans often combine HA filler for immediate restoration with Sculptra for long-term volume rebuilding. The result lifts the entire upper face.

How we treat the temples

Temple injection is advanced work. We use either deep needle technique against the periosteum or cannula placed in the fat pad layer, depending on anatomy and goals.

Product choice matters. HA fillers like Juvederm Voluma and Restylane Lyft provide immediate structure. Sculptra builds volume gradually across a series of treatments and is often the choice for clients who want slower, more gradual restoration. Radiesse is occasionally used in this area when more lift is needed.

Dosing is generous compared to other facial areas. The temple holds significant volume. Most plans use a half to full syringe per side, sometimes more for clients with significant hollowing.

Results from HA filler are immediate. Results from Sculptra develop over twelve to sixteen weeks. Most clients return for assessment in two to three months.
